School and/or district leadership must approve and sign the School Leadership Permission Form before the school can enroll in the SAFE School Program.

Stock Inhaler Event Form

The Stock Inhaler Event Form must be completed whenever a trained school staff member administers the stock albuterol inhaler.

The law requires the event is documented within 24 hours.
